Access Health Adams County supports Community Service Link, a web-based software system that facilitates a no wrong door access to social and health services in Adams County. Community Service Link is a community-wide information and client intake system that supports needs identification, service referral and enrollment, and client management, while minimizing the need for multiple client inquires and referrals. Using Community Service Link, an agency can use a single intake process to connect clients with multiple agencies services including access to healthcare programs. Care management services include client advocacy, connection to community resources, coordination of healthcare services in the PLAN, follow-up with PLAN patients after medical appointments, assistance with applications for Medicaid, MedAssist, Community Outreach Clinic, and other programs, facilitate referrals to CareLink for nursing care management when appropriate for PLAN patients, provide or connect clients with appropriate disease education when possible to ensure patient compliance and success, managing PLAN patient appointments and ensuring eligibility to physician offices, and provide appointment reminders for PLAN patients. The Voluntary Network of Providers is an innovative physician volunteer initiative to expand access to medical services for the low income, uninsured, medicaid insured residents of Adams County. It creates capacity for an ongoing relationship with individuals. The network builds on the capacity for care of the uninsured already provided by a number of providers, but adds a process for establishing client eligibility and ability to pay on a sliding scare, as well as a system for equitable distribution of uninsured patients.
